Social Care Management

NHP attempts to reach newly enrolled members by phone and/or by mail to conduct a Health Needs Assessment. The assessment gathers information about members' medical, behavioral, and psychosocial needs.

If needed, members are referred to NHP's internal care management programs where care managers work collaboratively with a member in order to increase positive health outcomes. Oftentimes, a member is followed by specific medical and/or behavioral care management programs along with social care management to help improve all aspects of a members' health and well-being.

NHP has a team of Social Care Managers (SCM) with extensive experience in the field of social and human services. Social care managers conduct assessments of NHP members and collaborate with other care managers to help meet the members' medical, behavioral, and psychosocial care management needs. In addition to coordinating with other internal programs, the SCM team helps members identify and procure community-based services such as income programs, housing, food, and transportation. The team can provide phone numbers, help with placing calls, help with applications, and can write referrals as needed, based on programs available.
